Hello,

I recently became interested in firearms again and would like to purchase a pistol for range shooting.

Here's the problem(s):  I only just found out that you have to change adresses on the fid card within 30 days.  We'll it's been about 12 years since I moved and still have the previous address on the card. Next, I was temporarily (1 night) confined to a health care facility for observation and relesed the next day. I had mixed alcohol with sleeping pills(was sleep deprived and trying to get to sleep), and got so sick my wife called an ambulance. So now I have to answer yes to question 18 on the applications
(change of adddress and pistol permit). Also, upon just finding my original ID card, on the back it states I have to surrender the card due to the reason stated above. Whew, should I just forget about this dream and move to Pennsylvania.

A few issues you raised: 

1- Change address within 30 days:  Yes, law states that, but it does not technically invalidate the FID.  Most dealers will not sell to you unless both the FID and DL matches, but you will note the law specifically states the FID does not expire unless you become subject to a disability specified in the statute [see below though].

2- Commitment:  The law (2C:58-3. Purchase of firearms) states no FID or handgun permit shall be issued=

(c)(3) To any person who suffers from a physical
defect or disease which would make it unsafe for
him to handle firearms, to any person who has
ever been confined for a mental disorder, or to
any alcoholic unless any of the foregoing persons
produces a certificate of a medical doctor
or psychiatrist licensed in New Jersey, or other
satisfactory proof, that he is no longer suffering
from that particular disability in such a manner
that would interfere with or handicap him in the
handling of firearms; to any person who knowingly
falsifies any information on the application
form for a handgun purchase permit or firearms
purchaser identification card;

3- Regarding failure to surrender the card, the law states under subsection (f) states=

firearms purchaser identification card
shall be valid until such time as the holder becomes
subject to any of the disabilities set forth
in subsection c. of this section, whereupon the
card shall be void and shall be returned within
five days by the holder to the superintendent,
who shall then advise the licensing authority.
Failure of the holder to return the firearms purchaser identification card to the superintendent
within the said five days shall be an offense under
section 2C:39-10a.

Although YMMV and you should seek counsel from an attorney, the concern I would have if I were you was the failure to surrender the card when you were committed (not necessarily the failure to change address within 30 days), and also the concern whether the PD would issue a duplicate.

More likely than not, if you didnt make any purchases after the card technically become revoked due to the commitment, you would not have a problem.  Also, if the facts are as you stated, you would answer YES on the app re commitment and then they would investigate.  You will have to get a cert from an MD as stated above.

Id consult a lawyer if I were you.

I wouldnt even worry about that mental health incident AS LONG as there is nothing in there stating you were a harm to yourself or others, like you were not trying to kill yourself. Just make sure you include the dates and hospital on your application, this process may take a while as they will contact the hospital to get records, review them and then you should be granted your FID card. I was committed for observation when i was about 11 for a week or two, it was all ADD related and I had "thrill seeking" behavoirs or some shit..you know just kid shit, but the pd still reviewed all the files but then said i wasnt a danger to myself or anything and granted me my FID. Every town is differnt though that was in bayville, i moved to TR and am not planning on buying any guns so i might hold off on address change before i consult evan nappen, i dont wanna fu*k myself if i dont have to you know what im saying?
